NYS OVS Claim Application (English and Spanish Versions) – this is a comprehensive claim form that victims can use to apply for all OVS compensation benefits for which they may be eligible, including essential personal property, personal injury, and/or death benefits.
A Guide to Crime Victim Compensation in NYS Brochure (English and Spanish Versions) – this brochure provides information on OVS compensation and answers frequently asked questions.
A Guide to Restitution in NYS (English and Spanish Versions) – this brochure helps victims understand the process of restitution and answers frequently asked questions.

OVS Annual Reports provide information on the agencies services and accomplishments throughout the fiscal year. The following information can be found in the annual reports: agency organizational chart, powers and duties of the Office, explanation of the claim process, eligibility requirements, forensic sexual assault information, various claim decision data, workload summaries, expenditure charts, vocational rehabilitation expenditures, public awareness and outreach, grants unit information, statewide conference synopsis, listing of victim/witness assistance programs, statewide advisory council, legal and statistical information, Son of Sam information, and data on restitution and subrogation.
